HOW TO REQUEST APE/ECU LETTER OF GUARANTEE (LOG)?

Letter of Guarantee for APE/ECU can be requested by sending us an email to APE@etiqa.com.ph

In your email, Please provide the following information: 

• Complete name:
• Card number:
• Company name: 
• Birthdate: 
• Contact number: 

Date of Availment Reminders:

• We usually allocate ten (10) working days to schedule clinic-based Annual Physical Examination (APE) to coordinate with the provider. Then, after the expected confirmation from the provider, we will send the Letter of Guarantee (LOG) within five (5) working days.

• We usually allocate twenty (20) working days to schedule Executive Check-up (ECU) to coordinate with the provider. Then, after the expected confirmation from the provider, we will send the Letter of Guarantee (LOG) within five (5) working days.

• APE/ECU can be availed depending on the medical coverage/benefit
