HOW TO REQUEST LETTER OF GUARANTEE

There are 2 ways to request Letter of Guarantee. 



1.) Via Smile PH app

  • Download the latest version of Smile PH app on Google Play Store (https://play.google.com/store/apps/details?id=ph.com.av_asian_life) or Apple App Store (https://apps.apple.com/ph/app/smile-ph/id1488302294)
  • Register your account details 
  • Key Functionalities includes:
    • View your Virtual Card
    • Request Letter of Guarantee (LOG)
    • File a Claim
    • Check your previous records
    • Send an Inquiry
    • Find accredited providers and doctors


2.) Via Member's Portal

  • Member's portal is a web-based portal created to make your plan details accessible on your browser. It has been designed with a clean and easy to navigate interface. 
  • Register to Member's Portal by visiting the website at:  https://portal.etiqa.com.ph/member/
  • After your successful log in, You will have an access to Key functionalities similar to Smile PH app.


Note: 

  • If you already have a Smile PH app Account, You need to use that credential in Logging in to Member's Portal or Vise Versa
  • Turn around time of Letter of Guarantee (LOG) for Laboratory request is within 4 Hours. 
  • Consultation LOG shall be issued within 15 minutes from the time of request by Auto LOG capabilities of our App/Portal.
